Adults can begin enrollment by downloading and completing the following volunteer documents:

1. Volunteer Pre-Enrollment Form

2. Volunteer Pre-Interview Questionnaire

3. Authorization for Background Check

Adults can also call SIRSS at 618-457-6703 (Jackson County) or 618-357-2513 (Perry County) and ask for:

• Kim Leonard at Extension 297,

• Crystina Wells at Extension 385, or

• Lisa Wait at extension 266.
