Assertive Community Treatment (ACT)

Assertive Community Treatment (ACT) is an around-the-clock approach that assures access to a multidisciplinary team that supports persons living with severe and persistent mental illness in the community.  This evidence-based service is nationally recognized as effective in reducing hospitalizations for persons who have not responded well to traditional approaches.

Services include 24-hour residential, intensive outreach/case management, diverse therapies, independence skill acquisition, money management, community integration, and medication monitoring and training.

How to Apply – This service is only available to persons enrolled in other SIRSS services.
