Before we can open your case for services, we are required to complete a financial statement. In order to do so, we will need certain documents and would appreciate your bringing in any of the following items that apply to you:
1. Social Security Number or card
2. Pay stubs (of household or family income)
3. SS or SSI, SSD information
4. Medicare, Medicaid or insurance card
5. Child support information
6. Court appointed legal guardianship documents
7. Statement of school grants/awards
8. Any other documents of income (see below)
If you have a current Medicaid (Public Aid) card, please bring it with you so that we can get a copy. If not, we ask that you bring a $5.00 co‑payment to your appointment. For those attending college, please bring financial information from the school you are attending such as student loans, grants, or financial aid. Please note, that if someone else claims you on their income taxes, such as your parents, we will need documentation of their income.
If you have no income it is important that you bring a notarized statement from the person providing for your current living expenses such as room and board. This can be done at most banks for a small fee. If you prefer, you can bring this individual in with you and our notary will provide this service.
