Information the Customer Care Representative will ask

When you call for an appointment the Customer Care Representative will ask you for the following basic information:

  • Name
  • Address
  • Phone Number
  • Social Security Number
  • Date of birth
  • What problems are you having at this time?
  • Will you need assistance in communication or appointment on the first floor?
  • Do you have Medicare, Medicaid, Insurance, Self-Pay, or EAP?
  • Who is referring you?

 

If you have questions just ask the Customer Care Representative who is assisting you.
