Assertive Community Treatment (ACT)

ACT provides an around the clock team approach toward supporting those adults with severe and persistent mental illness with the support and guidance they need to maintain housing and stay well. This evidence-based service is nationally recognized as effective in reducing hospitalizations for persons who have not responded well to traditional approaches.

Services include intensive outreach/case management, diverse therapies, independence skill acquisition, money management, community integration, vocational/educational support, and medication monitoring and training.
